We are recruiting on behalf of our client for an experienced and proactive Care Coordinator to support the safe, compliant, and high-quality delivery of community-based care services. This is a key operational role, ideal for an experienced Senior Carer or Care Coordinator looking to take on greater responsibility across service coordination, compliance, and staff support.

 

About the Role

As a Care Coordinator, you will oversee the day-to-day coordination of care services within the community, ensuring care delivery is safe, responsive, and aligned with regulatory standards.

You will act as a central point of contact for care staff, supporting workforce coordination, rota management, and service delivery. Working closely with the Service Lead, you will help maintain service quality, support staff in the field, and ensure the service remains inspection-ready.

The role combines coordination, quality assurance, and hands-on operational support, including audits, safeguarding processes, and responding to short-notice staffing needs.

 

Key Responsibilities

  • Coordinate care delivery across community-based services, ensuring continuity and quality of care
  • Manage rota planning, staffing allocation, and short-notice changes
  • Provide support and guidance to care staff, acting as a key point of contact
  • Conduct quality assurance activities, including spot checks, medication audits, and compliance monitoring
  • Support safeguarding, incident reporting, and risk management processes
  • Assist with care plan reviews, assessments, and service user reviews
  • Respond to emergency cover requirements, including covering care calls where necessary
  • Participate in the out-of-hours on-call rota
  • Maintain accurate records, rotas, and compliance documentation
  • Support continuous improvement and ensure the service remains inspection-ready

 

About You

  • Previous experience as a Care Coordinator, Senior Carer, or similar role within domiciliary care
  • Strong understanding of rota management, care coordination, and service delivery
  • Knowledge of CQC standards, safeguarding, and compliance processes
  • Experience conducting spot checks, audits, or staff supervision is desirable
  • Highly organised with the ability to manage multiple priorities
  • Confident communicator with strong problem-solving skills
  • Ability to respond effectively in a fast-paced, operational environment
  • Full UK driving licence and willingness to travel within the community
