The service supports people transitioning from hospital, secure and residential settings into appropriate community-based support. As the service continues to grow, this is an exciting opportunity to join an established team and play an important role in the development and smooth running of the service.

About the Role

As a Care Coordinator, you will be responsible for coordinating day-to-day staffing and service delivery, ensuring clients receive the right support and clinicians are appropriately matched to their needs.

You will be the first point of contact for a range of client and clinician queries, providing a responsive and professional service while ensuring all staffing arrangements are delivered safely, efficiently and in line with company procedures and care standards.

You will work closely with the Transforming Care, Account Management and Sales teams, supporting effective communication across the organisation and contributing to positive client relationships and the continued growth of the service.

The role is hybrid, with more office-based working encouraged during the first five months to support training, development and integration into the team.

There is no on-call requirement for this role.

  • Coordinate staffing solutions to meet client and service requirements.
  • Match clinicians’ skills, experience and availability to individual client needs.
  • Manage incoming queries from clients and clinicians, ensuring they are responded to promptly and professionally.
  • Work closely with internal teams to ensure smooth and compliant service delivery.
  • Maintain accurate staffing, client and clinician records.
  • Ensure all activity follows company policies, procedures and relevant care and compliance requirements.
  • Build and maintain positive working relationships with clients, clinicians and colleagues.
  • Support the wider team with service development and continuous improvement.
  • Contribute to the growth of the service by providing a high-quality and responsive coordination function.
